  • Education and Service Promotion

  • Provide education and/or training to Trillium Health staff regarding HIV prevention

  • Has the ability to train team members on walk-in testing services including HIV, HCV and STI testing.

  • Participate in outreach efforts to increase community awareness surrounding regarding HIV prevention services at Trillium Health

  • Participate in community HIV prevention educational sessions

  • Serve as an ambassador of all services of Trillium Health

  • Service Delivery

  • Work in conjunction with Trillium Health staff to identify current patients appropriate for PrEP/PEP

  • Educate potential patients about HIV prevention and facilitate direct linkage to Trillium Health PrEP/PEP services

  • Assess patients for barriers to adherence to appointments and medication and develop plans to reduce barriers

  • Work in collaboration with the Prevention Navigator 3 to screen and assist patients in applying for assistance to cover medical visits, laboratory fees and medications (i.e. NYS PrEP-AP, Trillium Health’s Sliding Fee Scale Program, Patient Assistance Programs for medication coverage, Prior Authorizations through third party payers to medication coverage)

  • Educate current patients about PrEP/PEP use, side effects, safety, and discontinuation criteria, under the guidance of a provider or clinical pharmacist

  • Facilitate linkage to additional services needed (internal or external) as indicated by screening or assessment (including, but not limited to primary care, psychosocial and other supportive services)

  • Testing

  • Responsible for providing HIV/HCV and STD screening and testing services, as per protocol, for walk-in clients, patients, special events and community testing events

  • Conduct a behavioral risk assessment, assessing risk for HIV, HCV and STDs

  • Connect client to clinical and/or pharmacy services if they are a candidate for Post- or Pre-Exposure Prophylaxis

  • Ensure the client is aware of how HIV/HCV/STDs are transmitted, how to reduce their risk and/or prevent transmission

  • Discuss the meaning of a nonreactive and reactive test result

  • Conduct rapid HIV, STI and HCV testing per protocol

  • Discuss strategies for risk reduction, including Pre-Exposure Prophylaxis, and assist client in creating a practical risk reduction strategy to employ following the test encounter

  • Provide risk reduction supplies to client/patient, as needed

  • Demonstrate developing knowledge of HIV/HCV/STDs, factors affecting people who use drugs, health disparities and other health-related issues relevant to the mission and vision of Trillium Health

  • Be an ambassador for all Trillium Health services during community based outreach, education and testing

  • Document client encounters and services delivered in the electronic medical record

  • Document testing services provided as required by funders

  • Administrative

  • Enter, compile and report program data as required by program and/or funder

  • Function as liaison with internal staff, pharmacies, home care and other community agencies to promote linkage, retention and treatment adherence services for PrEP/PEP

  • Work with the Clinical Informatics, Quality and Business Intelligence teams to review data accuracy and perform quality improvement projects as needed

  • Maintain inventory of testing supplies, HIV prevention tools, patient education materials and incentives

  • Participate in training and continuing education initiatives to build understanding of evolving treatment and service delivery issues

  • Assist in the onboarding and training of staff including documentation in the EMR for program specific activities

  • Is capable of staffing the walk-in testing calendar as needed

  • Able to provide quality assurance for all HIV/STI/HCV related testing and documentation

  • Requires the ability to relate to people of diverse backgrounds, cultures, races, sexual orientations and gender identities or expressions

  • Responsible for maintaining confidentiality of all patient, clients, proprietary, and protected information

  • Must have a reliable transportation; preferred, but not required, is a valid NYS driver’s license and proof of required automobile insurance

  • Employees are accountable for meeting performance standards of their departments. They participate in compliance audits and quality improvement plans.

  • Other specific duties as assigned.

  • It is preferable if employees are able to work flexible hours including weekdays, evenings and Saturdays.

Qualifications

Bachelor’s degree in health education or related field preferred. Knowledge of Pre-Exposure Prophylaxis required; knowledge of HIV testing, prevention and counseling, as well as EMR and AIRS data entry experience preferred. Bilinguals in Spanish and/or ASL preferred. Employee must be able to work flexible hours including weekdays, evenings and Saturdays.

Physical Requirements

While performing the duties of this job the employee is required to stand, sit, walk, use hands to finger, handle, or feel; reach with hands and arms, talk and hear. Occasionally the employee must stoop, bend and lift or move up to 25 lbs. Specific vision abilities required include close vision, distance vision, peripheral vision, depth perception and ability to adjust focus.
