Transitional Living Services of Northern New York Residential Case Manager - Ogdensburg in Ogdensburg, New York
Provides ongoing support to residents so that they may acquire the necessary skills to live independently in the community. Performs a wide range of tasks including but not limited to training in areas of self-advocacy/assertiveness, daily living skills, community integration, medication and symptom management, skills development, socialization, and substance abuse. Converses with residents to reinforce positive behaviors and to promote social interaction as a role model. Services are individualized to the specific identified needs of each person in a culturally sensitive way. This is achieved using a holistic approach considering several areas of strengths and needs including, but not limited to the individual’s: history, culture, spiritual preferences, education and employment information, social involvement, mental health and substance abuse status, medical status, safety issues, legal issues and financial issues. Observes and documents resident’s behaviors to help facilitate assessment of goals. This position may require use of personal vehicle to transport clients when an agency vehicle is not available.
ESSENTIAL DUTIES AND RESPONSIBILITIES
Participates in the oversight of the maintenance of the residence and its conformity to health and safety requirements.
Follows agency policies and procedures in all program areas.
Serves as manager of all services the person will receive under the case management program.
Develops written case management plan based on strengths and individualized needs.
Reviews, revises and updates the case management plan every 90 days or as needed.
Links person to all services and supports listed on the individualized case management plan.
Advocates on behalf of the individual to gain access to needed services and supports in the least restrictive setting.
Maintains adequate and appropriately written progress notes in the case file according to agency guidelines
Develops and maintains good community relationships.
Assists program participants with rehabilitation, social, employment and health supports.
Assists in daily living skills development.
Assists program participants in times of crisis.
Coordinates treatment with other agencies.
Increases use of appropriate community resources.
Encourages participation in client’s own case management plan.
Commences to develop a discharge plan upon enrollment in the program.
Conducts regular record keeping reviews and completes audits according to procedure.
Assists in reviewing, evaluating, and further development of the case management program.
Monitors and maintains high standards of service delivery.
Collects data for statistical purposes.
Maintains client confidentiality at all times.
Reports to supervisor pertinent information concerning illnesses, accidents, untoward events, staff difficulties, repair needs, etc.

Lead Residential Case Manager
- Field resident concerns and day-to-day SRO issues in the absence of the Director of Operations
- Oversee completion of quarterly, annual or special reports
- Coordinate representation at all SPOA meetings
- Conduct community outreach to bring local organizations on site to provide enriching activities for residents (not new, just re-written)
- Provide coverage at other locations when the assigned case manager is absent
- Oversee on-site orientation of new case managers
- Responsible for development of materials used in on-site programming and ensuring consistent implementation across sites
- Complete a case record review on a sample of SRO casefiles each quarter and ensure any necessary corrections are made
- Represent CHAAP as a member of the Program Committee
- Attend quarterly Quality Assurance Reviews
- Coordinate follow-up from Quality Assurance Site visits and documentation reviews
Experience and Skills:

EDUCATION and/or EXPERIENCE
Bachelor’s degree in the Human Services field; and related experience and/or training working with seriously and persistently mentally ill adults, or an Associate’s Degree in the Human Services field and three years’ experience in human services, or a High school diploma or general education degree (GED); and five years related experience and/or training in a human service filed. Preferred: Experience with developmentally delayed, geriatric and co-occurring disorder clients.
Ability to read and comprehend simple instructions, short correspondence, and memos. Ability to write simple correspondence. Ability to effectively present information in one-on-one and small group situations to clients, and other employees of the organization.
Ability to add, subtract, multiply, and divide in all units of measure, using whole numbers, common fractions, and decimals. Ability to compute rate, ratio, and percent and to draw and interpret bar graphs.
Ability to apply common sense understanding to carry out detailed but uninvolved written or oral instructions. Ability to deal with problems involving a few concrete variables in standardized situations.
CERTIFICATES, LICENSES, REGISTRATIONS
Must maintain valid driver’s license. Must maintain current Standard First Aid and Cardiopulmonary Resuscitation certifications.
Individuals must perform his or her duties in a safe and effective manner. Individuals must report any unsafe working condition or practice immediately including any injuries or illness.
Individuals are required to attend annual Corporate Compliance training. Individuals will be required to properly maintain complete, accurate, and contemporaneous records as required by the Agency. All work is to be completed with honesty and integrity and in compliance with HIPAA requirements. Individuals should refrain from engaging in conduct that conflicts, or is perceived to conflict with the best interest of the agency.
PHYSICAL DEMANDS While performing the duties of this job, the employee is regularly required to stand; walk; use hands to finger, handle, or feel; talk or hear; and taste or smell. The employee frequently is required to sit and reach with hands and arms. The employee is occasionally required to climb or balance and stoop, kneel, crouch, or crawl. The employee must regularly lift and/or move up to 25 pounds and occasionally lift and/or move up to 50 pounds.
While performing the duties of this job, the employee may be exposed to outside weather conditions. The noise level in the work environment is usually moderate. May be exposed to situations in which unpleasant physical, emotional, or behavioral conditions exist. Use of tobacco products by employees is not permitted at any TLS property or work site to include agency vehicles.
