Job Description

Provides drug information and assists prescribers in providing optimal pharmaceutical care for patients. Completes required learning experiences and select elective learning experiences, assigned residency projects, and educational competencies, goals and objectives as required by the individual program and as defined by the American Society of Health-System Pharmacists.

Responsibilities Include:

  • Develops the program competencies designed to achieve the following Post Graduate Year (PGY) goals PGY1

  • In collaboration with the health care team, provides safe and effective patient care to a diverse range of patients following a consistent patient care process.

  • Ensures continuity of care during patient transitions between care settings.

  • Prepares, dispenses, and manages medications to support safe and effective drug therapy for patients.

  • Demonstrates ability to manage formulary and medication-use processes, as applicable to the organization.

  • Demonstrates ability to evaluate and investigate practice, review data, and assimilate scientific evidence to improve patient care and/or the medication use system.

  • Demonstrates leadership skills.

  • Demonstrates management skills.

  • Provides effective medication and practice related education to patients, caregivers, health care professionals, students, and the public.

  • Employs appropriate preceptors' roles when engaged in teaching.PGY2

  • Exhibits ongoing development of essential personal skills of a practice leader.

  • Establishes oneself as an expert for medication-related information and resources within an organization.

  • Contributes pharmacy specialist's perspective to an organization's technology and automation systems decisions.

  • Contributes to the leadership and management activities within the practice area.

  • Exercises practice leadership.

  • Evaluates core biomedical literature resources appropriate for the specialized area of pharmacy practice.

  • Develops collaborative professional relationships with members of the health care team.

  • For aassigned patient caseload, prioritizes delivery of pharmaceutical care.

  • Establishes collaborative pharmacist-patient and pharmacist-caregiver relationships.

  • Collects and analyzes patient information.

  • Triages patients.

  • Designs evidence-based therapeutic regimens.

  • Designs evidence-based monitoring plans.

  • Recommends regimens and monitoring plans.

  • Designs education for a patient's regimen and monitoring plan.

  • Implements regimens and monitoring plans.

  • Evaluates patient progress and redesigns related regimens and monitoring plans.

  • Communicates pertinent patient information to facilitate continuity of care.

  • Documents direct patient-care activities appropriately.

  • Employs advanced literature analysis skills in preparing drug information.

  • Provides pharmacy expertise to the organization in the review of existing, development of new, and implementation of the organization's policies and procedures affecting the care of patients.

  • Identifies opportunities for improving the safety of aspects of the organization's medication-use system.

  • Assists organization in achieving compliance with accreditation, legal, regulatory, and safety requirements related to the use of medications

  • Leads review of existing, and development/implementation of new medication-related guidelines/protocols for the care of patients served by the specialized area of pharmacy practice.

  • Provides effective education and training to health care professionals and health care professionals in training.

  • Designs and delivers education programs to the public that center on health improvement, wellness, and disease prevention.

  • Conducts a pharmacy-related research project using effective research and project management skills.

  • Engages in the publication process.

  • Performs related duties, as required.

Qualifications

PGY1

  • Doctorate of Pharmacy (PharmD) from an Accreditation Council of Pharmacy Education (ACPE)-accredited school of pharmacy, required.

  • Eligible for pharmacist licensure in New York State.PGY2

  • Doctorate of Pharmacy (PharmD) from an Accreditation Council of Pharmacy Education (ACPE)-accredited school of pharmacy, required.

  • Eligible for pharmacist licensure in New York State.

  • Completion of a ASHP-accredited Post-Graduate Year 1 (PGY1) residency program, required.
