Family Health Centers at NYU Langone, a designated Level 3 Medical Home, is the largest federally qualified health center network in New York State, and the second largest in the nation. It includes nine primary care sites, 33 school-based health centers and dental clinics, four day care centers, the nation's largest dental residency program, and New York State's largest behavioral health program. The network also includes 12 community medicine sites providing care to disenfranchised New Yorkers, comprehensive HIV services, chemical dependency programs, and a family support center that offers educational, vocational, and other social support programs. Position Summary:
We have an exciting opportunity to join our team as a Health Educator (WIC/Ambltry). The health educator will facilitate evidence based sexual health workshops for youth, implement risk reduction counseling and provide sexual health education at Wingate Campus High School. They will work closely the SAMHSA initiative team and Project SAFE.
Job Responsibilities:
Conduct evidence-based sexual health workshops in Central Brooklyn high schools and community based organizations using the Making Proud Choices curriculum.
Administer substance abuse and sexual health risk assessment surveys to all workshop participants.
Meet individually with workshop participants identified as "high risk" to provide risk reduction counseling using Motivational Interviewing.
Connect workshop participants to FHC School Based Health Center, FHC Flatbush Health Center, or other social services.
Promote FHC Flatbush Health Center services.
Provide sexual health education to youth at Wingate School Based Health Center.
Provide rapid HIV counseling at Wingate School Based Health Center and complete all related documentation.
Participate in FHC community events to connect youth STI and HIV testing.
Participate in weekly staff meetings at Project Reach Youth.
Submit all required documentation.
Minimum Qualifications:
To qualify you must have a HS Diploma. Prior work experience with community health outreach. Health related background.
Qualified candidates must be able to effectively communicate with all levels of the organization.
